Where would you rank your athlete’s Flexibility?
When it comes to your athlete moving their body, are they able to hold different positions and form shapes (ie touching their toes, sitting or standing)? Do they have any injuries that have been caused from a lack of flexibility/mobility?
1 means your athlete has limited/no flexibility (can’t touch their toes). 5 means your athlete is extremely flexible/mobile.
Where would you rank your athlete’s Strength?
When it comes to body weight exercises, can your athlete perform with good form: push-ups, squats, sit-ups?
1 means your athlete cannot perform bodyweight exercises. 5 means your athlete can perform multiple reps with great form.
Where would you rate your athlete’s speed?
Are they slower than kids their age? Or are they able to keep up with their friends and/or teammates on the playground and/or field? If they have participated in timed speed testing, where do they rank against their team?

Do they make contact with the ball more often than not? Or are they consistently swinging and missing pitches?
1 means they strike out a lot and rarely make contact. 5 means they make contact with the ball the majority of the time.
Where would you rate your athlete’s ability to hit for power?
Do they usually have more extra base hits than the other guys on their team? Or do they have trouble getting the ball out of the infield?
1 means they rarely hit the ball out of the infield. 5 means that when they hit the ball they usually get a double, triple or home run.
Where would you rate your athlete’s effort, attitude and coachability?
How do they act when being coached? When in groups, are they able to focus on the coach and execute what he is asking? When your athlete is tired, do they continue giving their all or do they quit?
1 means they easily give up and/or get distracted. 5 means they are self-motivated and push through anything.
